Schedule a Mediation

There are several ways you can schedule a mediation with Max Weiss Mediation. 

Online Form

We have prepared an online form that allows you to simply fill in the necessary information and submit it to our staff via the web. 

E-mail

Email us at settle-it@maxweissmediation.com with as much information about the case (parties' names and addresses, names of respective attorneys, etc.) and our staff will contact you and schedule a convenient time and location for the mediation.

Complete our Mediation Submittal Form (Download PDF file) and fax it to us  (814.336.6510).
